Five Wishes Workshop and Local Resources for Seniors

Join on Wednesday, November 9 from 4-6 pm for an informational session with Dawn Stewart, Gunnison Valley Health Home Medical Services.

In the first hour she will cover Home Medical Services and what senior care looks like currently in the valley.  In the second hour she will facilitate the Five Wishes workshop described below.

 Come learn more about this valuable FREE planning tool for advanced directives and end of life care. The Five wishes document is a great conversation starter for individuals and families to outline wishes for medical treatment, how they want to be treated at end of life and final arrangements.  The document is easy to complete and with 2 witness signatures is legally valid in the state of Colorado.  In this workshop we will review each option, answer questions and if participants would like complete their own Five Wishes and have it witnessed.

About Dawn:

Dawn Stewart, LSW works for GVH’s Home Medical Services as their Medical Social Worker and Bereavement Coordinator.  Dawn works closely with nurses, physical and occupational therapists to help patients find the resources they need in the community, support families through the Hospice expereince and work through grief and loss.  She earned her Master’s Degree in Social Work from the University of Kansas in 2004 and has a had a varied social work career since then including work with sexual and domestic violence survivors, adults with developmental disabilities and children in foster care. Dawn and her family have been here in the valley for three years and she is grateful for the opportunity to serve the community through Home Health, Hospice and Palliative Care.
